Offer available only to Service Providers that meet Facilitize’s onboarding criteria. One offer per Service Provider.
To activate: sign the pilot order form and provide a $200 match; Facilitize applies a $200 promo ad credit at launch.
Month-1 budget: campaign runs on a combined $400 pool. After it’s spent, Service Provider funds 100 % of future media unless a new order form is signed.
Management fees: waived in Month 1. From Month 2, a $250/mo fee applies while ads are active; cancel ads anytime. If ads are cancelled but Facilitize FSM remains, the license reverts to $60/mo.
Lead cap & license: once the agreed qualified-lead cap is reached, the Facilitize FSM license fee of $50/mo begins.
Credit expiry: promo credit applies to future ad costs only and expires at pilot end; unused credit has no cash value.
Tracking: Facilitize platform data is the definitive record for spend and lead attribution; both parties will reconcile any off-platform leads in good faith.
Cancellation: either party may cancel with 7 days’ written notice; earned but unpaid fees remain due, and campaigns pause at the end of the billing cycle.
No guarantees: Facilitize makes no guarantee of lead volume, revenue, or ROI; neither party is liable for indirect or consequential damages.
Miscellaneous: Nevada law governs. Facilitize may withdraw the offer for non-payment or breach.
